Application Procedures & Forms
- Choose the scheme — identify the accreditation scheme (e.g. ISO/IEC 17025 testing, ISO/IEC 17020 inspection, ISO 15189 medical laboratory, ISO/IEC 17065 product certification, ISO/IEC 17021-1 management systems, ISO/IEC 17024 personnel) on the Accreditation Schemes pages.
- Submit the application form — the downloadable application form and the annexes (scope sought, organisation chart, personnel list, equipment list, quality manual index) are available in the Document Library and through the online application form.
- Administrative review — completeness is checked within 10 business days; missing items are requested in writing.
- Quotation & contract — assessment costs are quoted per the Fee Schedule; the terms & conditions are signed before the technical assessment starts.
- Conflicts of interest — the applicant may object to specific proposed assessors on documented conflict-of-interest grounds; the objection is handled by the technical secretariat and does not prejudice the application.
